Prophylactic administration of Paracetamol/Acetaminophen at the time of vaccination should not be routinely recommended since antibody responses to several vaccine antigens (pneumococcus, Haemophilus, diphtheria, and tetanus)are reduced. It should be given when symptoms actually develop (Czech childhood vaccine study).It is known that the acellular pertussis vaccine is less immunogenic than the whole cell vaccine. If this is already the case, adding prophylactic Paracetamol can lower the immune response even further. In a child with history of febrile convulsion or a severe reaction to an earlier dose of the same vaccine one may use paracetamol as prophylaxis.
