In first 48 hours, many cyanotic congenital heart diseases can be easily missed. To avoid that, we must assess saturations before discharging the child- at least in one upper and one lower limb. In cyanotic heart disease, saturations are usually less than 95%. If the difference in SpO2 of upper and lower limbs is more than 5%, then we should get an echocardiogram before discharging the newborn.
