Regular packed cell transfusions are initiated when Hb is < 7gm/dl on 2 occasions more than 2 weeks apart (excluding all other contributory causes such as infections, bleeding etc.). In addition, transfusion therapy is also recommended for those with Hb > 7 gm/dl who have gross facial changes, poor growth, fractures or extramedullary hematopoiesis.

  • Pre-transfusion Hb should be maintained between 9-10.5 gm/dl, as it eliminates the complications of anemia and suppresses intestinal absorption of iron.
  • Donor blood should be thoroughly screened preferably by nucleic acid testing (NAT).
  • Blood for transfusion should be leucocyte-depleted, preferably by pre-storage filtration.
  • Blood should be stored with additive solution like AS-1 (Adsol), AS- 3 (Nutricell) and AS-5 (Optisol) which allows longer storage upto 42 days.
